PROMOTION DESCRIPTION: This promotion is sponsored by Once Upon A Child (6946 N. Academy Blvd Colorado Springs, CO). This promotion begins at 9pm MT on Thurday, Dec 12th and ends at Sunday December 29th 4pm MT. Winner(s) will be chosen in two ways. One winner will be based on the most “Votes” for a chance to win a $100 gift card to Once Upon A Child. The other winner will be selected based on receiving the highest number of votes from our voting panel to win the Monster Trax Convertible. Winner(s) will be announced via E-Mail and Facebook. Promotion is void where prohibited. By entering, each entrant accepts and agrees to be bound by these promotion rules.

PRIZE(S): Winner with the most facebook “votes” will win a $100 gift card. In the case of a tie, the winners will split the $100 gift card evenly. The other winner will be selected based on receiving the highest number of votes from our voting panel to win the Monster Trax Convertible!
ODDS OF WINNING: Odds of winning will be determined by the number of entries received.
HOW TO PARTICIPATE IN THIS CONTEST:
Contestants can post a picture of their “Holiday Photo” entry on the Once Upon A Child Colorado Springs Facebook Page: https://www.facebook.com/OnceUponAChildCS/app_451684954848385 The “vote” total will only be counted from that point.
DISQUALIFICATION: Entries are limited to one per person. Multiple entries will result in disqualification.
ELIGIBILITY: Children between the ages of 0 and 12 are eligible to enter the contest. Children must have parent/guardian consent to participate in the contest. By entering to win the prize, the parent/guardian of the winner grants Once Upon a child permission to use his/her name, likeness, and picture for advertising and future promotion purposes without additional compensation (except where prohibited by law). Unaccepted prizes may not be awarded. This contest is subject to these complete and Official Rules.
NOTIFICATION: Prize winners will be notified by E-Mail, and Once Upon a Child is under no obligation to notify winners by any means other than E-Mail. Once Upon a Child is not responsible for notifications that are not received. If a potential winner cannot be reached after a reasonable effort has been made during 2 business days from the first notification attempt, such person may be disqualified, with an alternate winner selected. Any prize notification returned as undeliverable will result in the awarding of that prize to an alternate winner.
REDEMPTION: Once Upon a Child will deliver the prizes as follows: All Prizes will be available for pick up at Once Upon A Child 6946A N. Academy Blvd., Colorado Springs, CO 80918 between the hours of 9am and 7pm MST Mon-Sat, and 12pm-6pm on Sundays, unless otherwise specified. Prizes are non-transferable and no substitutions or cash equivalents are allowed except by Once Upon A Child, who also reserves the right to award a prize of equal or greater value if the advertised prize is unavailable.
CONDITIONS OF ACCEPTANCE OF PRIZE: Driver’s license or other appropriate picture identification is required to claim a prize. The winner(s), parent/guardians(s), and/or any other persons necessary may be required to execute and return a release and affidavit of eligibility within 5 days of prize notification, or the prize will be forfeited. A winner must take possession of his or her prize within 5 days or the prize will be forfeited. Winner(s) will be solely responsible for any and all local, state, and federal taxes, as well as license and registration fees incurred by participating in the promotion, including, but not limited to, transportation costs, gratuities and other items of an incidental nature. Prize(s) may not be exchanged for cash or any other consideration. Each winner agrees to the use of his or her name and likeness in publicity without any additional compensation, except where prohibited by law. By entering this promotion, each winner acknowledges that Once Upon a Child has the right to publicize and broadcast each winner’s name, voice, likeness, portrait/video, recorded voice and/or biological material, and the fact that he or she won, and all matters incidental thereto.
CONDITIONS: By participating in the contest, you agree to release and hold Once Upon A Child, Shideler Retail, Inc., and its directors, officers, employees and agents, and all prize suppliers/sponsors harmless from any and all losses, damages, rights, claims, and actions of any kind in connection with the contest or any product purchased, or resulting from acceptance, possession, use or misuse of any prize, including, without limitation, personal injuries, death, and property damage, and claims based on publicity rights, defamation, or invasion of privacy, whether suffered by you or a third party. This contest is void wherever prohibited or restricted by law and is subject to all applicable federal, state and local laws and regulations. By participating, entrants agree to be bound by these Official Rules and the decisions of Once Upon A Child and waive any right to claim ambiguity in the contest or these Official Rules. All decisions regarding the contest, including, but not limited to, selecting winners, will be made by Once Upon A Child , whose decisions shall be final. Winners are solely responsible for all applicable federal, state and local taxes.
